Will the Minister of HEALTH AND FAMILY WELFARE be pleased to state:
(a) the number of Ayushman Bharat Health and Wellness Centre established in the country, State/UT-wise and specifically in Pratapgarh, Deoria and Jhansi district of Uttar Pradesh, Pali district of Rajasthan and Dewas district of Madhya Pradesh;
(b) the total number of Ayushman Bharat Health accounts created, State/UT-wise and specifically in said districts;
(c) whether the Government is taking steps to disseminate the information about the said Scheme to the citizens; and

(a) & (b). As on 24.07.2023, a total of 1,60,480 Ayushman Bharat – Health and Wellness Centres (AB-HWCs) have been operationalized in India by upgrading existing SHCs and PHCs to deliver Comprehensive Primary Health Care (CPHC) that includes preventive, promotive, curative, palliative and rehabilitative services which are universal, free, and closer to the community.
As on 24.07.2023, a total 43,27,71,870 ABHA Numbers have been created.
State/UT-wise  details  of  AB-HWC  operationalised  alongwith  details  of  ABHA Numbers created as on 24.07.2023 is at Annexure.
Details of AB-HWCs in Pratapgarh, Deoria & Jhansi district of Uttar Pradesh, Pali district of Rajasthan and Dewas district of Madhya Pradesh alongwith details of ABHA Numbers created as on 24.07.2023 in these districts are as under.

(c) & (d): Ayushman Health melas are ‘one-stop’ platforms to address a comprehensive range of health issues have proven to be an effective strategy to reach the masses and augment the utilization of healthcare services. Melas held at the level of AB-HWCs to optimize utilization of health care services and generate awareness. The expected outcomes of these melas/ camps are increased access to specialized care, strengthen continuum of care, building the trust between the community and the health system, improved health-seeking behaviour and health literacy in the community.

(a) whether the Government has launched Ayushman Bharat – Health and Wellness Centres (AB-HWCs) initiative under the Ayushman Bharat Programme and if so, the details thereof along with achievements made since its launch;

(c) whether the Government has set parameters for setting up of PHCs and CHCs and if so, the details thereof along with the percentage of population covered by PHCs and CHCs during last three years;

(d) whether the Government has received proposals from States/UTs including Tamil Nadu for opening of new CHCs and PHCs and if so, the details thereof along with the funds sanctioned and proposed to be released for setting up of CHCs/PHCs in the country, State/UTwise; and

(e) whether the Government proposes to upgrade all the CHCs/PHCs as ISO certified wellness centres and improve quality of services and if so, the details thereof along with the number of ISO certified CHCs/PHCs functioning in the country, State/UT-wise?

STATEMENT REFERRED TO IN REPLY TO LOK SABHA STARRED QUESTION NO. 333* FOR 25TH MARCH, 2022

(a) to (e): Ayushman Bharat-Health and Wellness Centres (AB-HWCs) was launched in 2018 to progressively achieve Universal Health Coverage through delivery of Comprehensive Primary Healthcare Services. It envisages to transform 1,50,000 Sub-Health Centres (SHCs) and Primary Health Centres (PHCs), both Rural & Urban, into Health and Wellness Centres (HWCs) by December, 2022. As on 23rd March, 2022, a total 1,00,768 Ayushman Bharat-Health and Wellness Centres (AB-HWCs) are functional in the country which includes 73,668 SHC-HWC, 22,472 PHC-HWC and 4,628 UPHC-HWC, as reported by the States/UTs on AB-HWC portal. State/UT wise Central Release under Ayushman Bharat- Health & Wellness Centres (AB-HWC) under NHM from the FY 2018-19 to 2021-22 is given at Annexure-I.

As per RHS 2019-20, the present status and achievements of Community Health Centres (CHCs) and Primary Health Centres (PHCs), State/UT-wise, including Tamil Nadu, are given at Annexure-II.

Government of India has laid down Indian public Health Standards (IPHS) for services, infrastructure, human resource, diagnostics, equipment, medicines, etc. They are used as the reference point for planning and up-gradation of public health care infrastructure in the States and UTs. These IPHS have given population norms for establishing the public health facilities. As per RHS, 2019-20, average rural population covered by the health facility (based on the mid-year population as on 1st July, 2020) is given below

Besides this, States have the flexibility to propose for funding under the overall ambit of NHM for up-gradation of infrastructure, HR and services as per IPHS for public health facilities, after due gap analysis. States also have the flexibility to establish PHCs at 20,000 population and CHCs at 80,000 population in hilly/tribal/difficult areas.
All the States/UTs submit the proposals for opening of new CHCs and PHCs, through Program Implementation Plan (PIP) annually before the commencement of next financial year. Year-wise Record of Proceedings (RoPs) and PIPs of all the States /UTs under NHM, including Tamil Nadu, are available at the link  https://nhm.gov.in/index4.php?lang=1&level=0&linkid=59&lid=72.
The State of Tamil Nadu has submitted a proposal to MoHFW, GOI during PIP 2022-24 for establishment of 25 New Primary Health Centre and 25 New Urban Primary Health Centre under NHM.
 
The ISO 9001:2008, now ISO 9001:2015 standards were initially framed for the manufacturing industry and later adapted for service industry like healthcare industry. However, these standards are generic in nature. Therefore, the Ministry of Health and Family Welfare have formulated National Quality Assurance Standards (NQAS) in November 2013 to improve the quality of care delivered at different level of public health facilities. The Standards are accredited by apex international accrediting Agency ISQUA (International Society for Quality in Healthcare). Over the years, the programme has expanded its scope based on the emerging requirements and responses to newer initiatives taken up by the Ministry of Health and Family Welfare.
Quality Assurance Program has provision for national and state level quality certification of Health facilities on meeting the predetermined certification criteria. The program is being implemented across all states and UTs. As on 16th March, 2022, 1463 public health facilities are quality certified at the national level and 3576 health facilities at the NQAS certified at the State level.
The National Quality Assurance Standards are broadly arranged under 8 “Areas of Concern”– Service Provision, Patient Rights, Inputs, Support Services, Clinical Care, Infection Control, Quality Management and Outcome.

More than 10,000 Ayushman Bharat-Health and Wellness Centres become operational 
More than 1.3 crore screened for NCDs at the HWCs 
The first Health and Wellness Centre (HWC)under Ayushman Bharat was inaugurated by the Prime Minister Shri Narendra Modiat Jangla in Bijapur Chhattisgarh on 14th April 2018. Since then 10,252 HWCs have been operationalized. The states of Andhra Pradesh, Tamil Nadu, Uttar Pradesh, Karnataka and Kerala lead the way with the highest number of HWCs under Ayushman Bharat. Andhra Pradesh has operationalized 1361 HWCs, Tamil Nadu 1318, UP 912, Karnataka 700 and Kerala 678 HWCs.

A total of 1,33,84,332women and men, thirty years of age and above have been screened for common Non Communicable Diseases (NCDs) at these HWCs. The status of universal NCD screening,newly identified positive cases and individuals on treatment at HWCs is as follows:

The National Health Policy, 2017 recommended strengthening the delivery of primary health care, through establishment of “Health and Wellness Centers” (HWCs) as the platform to deliver Comprehensive Primary Health Care (CPHC).
Ayushman Bharat has two components which are complementary to each other. Under its first component, 1,50,000 existing Sub- Health Centres (SHCs) and Primary Health Centres (PHCs) will be transformed to Health & Wellness Centres (HWCs) to deliver Comprehensive Primary Health Care (CPHC), that is universal and free to users, with a focus on wellness and the delivery of an expanded range of services close to the community. The wide range of services provided at these Health and Wellness Centres will encompass maternal and child health services, communicable and non-communicable diseases, services for the elderly and palliative care including free essential drugs and diagnostic services. 
An expanded range of services will be provided at the HWCs, with the level of complexity of care at the PHC being higher than at that at SHC. States will also have the flexibility to expand the service package to address problems of local importance as defined by disease prevalence and community feedback.
In the urban context, the Urban Primary Health Centres or Urban Health Posts where they exist, would be strengthened to deliver comprehensive primary health care. The norm of One Multipurpose worker MPW-(F) per 10,000 population supported by four-five ASHAs, will enable outreach services, preventive and promotive care and home and community-based services. Therefore, in the urban context, the team of ANM and ASHA would be considered to equivalent to a frontline provider team with the first point of referral being the UPHC catering to about at 50,000 populations. 
HWC at the sub centre level are being staffed by suitably trained Mid- Level Health Provider (MLHP), also called as Community Health Officers (CHO) – who is a BSc/GNM Nurse or an Ayurveda Practitioner trained in primary care and public health skills and certified in a six months Certificate Programme for in Community Health. The training programme is being rolled out through IGNOU and State specific Public/Health Universities. 133 IGNOU Programme Study Centres (PSCs) in various district hospitals and 90 other PSCs under the state specific Certificate Programmes in the state of Maharashtra, Tamil Nadu, Gujarat and West Bengal have been notified, taking the total of Programme Study Centres to 223 PSCs across the country. 
All basic and post-basic nursing colleges and universities have been recently notified to integrate the training of MLHPs in their existing curriculum of nursing. This will enable production of 1,12,546 candidates annually that will be eligible to become CHOs. A total of 77691 ASHAs, 20024 MPWs/ANMs, 3837 Staff Nurses and 3548 PHC Medical Officers (1,05,100 in all) have been trained, so far. 3,54,22,026 individuals have been empaneled at HWCs.
Ayushman Bharat (AB), is India’s path to Universal Health Coverage. It signifies a move from selective health care to the delivery of a comprehensive range of services spanning preventive, promotive, curative (both outpatient and hospitalization) rehabilitative and palliative care. It aims to undertake path breaking interventions to holistically address health at primary, secondary and tertiary level. It is perhaps the single most important reform measure of Government of India, to address the major challenges of increasing prevalence of chronic diseases, high out of pocket expenses, and fragmented care delivery.

10,000 से भी अधिक आयुष्मान भारत – हेल्थ एंड वेलनेस सेंटर चालू हो गए हैं  

इन केन्द्रों में 1.3 करोड़ से भी अधिक लोगों में गैर संचारी रोगों की जांच की गई है 
आयुष्मान भारत के तहत प्रथम हेल्थ एंड वेलनेस सेंटर (एचडब्ल्यूसी) का उद्घाटन प्रधानमंत्री श्री नरेन्द्र मोदी द्वारा 14 अप्रैल, 2018 को छत्तीसगढ़ के बीजापुर स्थित जांगला में किया गया। उसके बाद से लेकर अब तक 10,252 हेल्थ एंड वेलनेस सेंटर चालू हो चुके हैं। आयुष्मान भारत के तहत हेल्थ एंड वेलनेस सेंटरों की सर्वाधिक संख्या की दृष्टि से आंध्र प्रदेश, तमिलनाडु, उत्तर प्रदेश, कर्नाटक और केरल इस मामले में सबसे आगे चल रहे हैं। आंध्र प्रदेश में 1361 एचडब्ल्यूसी, तमिलनाडु में 1318, उत्तर प्रदेश में 912, कर्नाटक में 700 और केरल में 678 एचडब्ल्यूसी हैं।
आयुष्मान+भारत+हेल्थ+एंड+वेलनेस+सेंटर

इन एचडब्ल्यूसी में 30 वर्ष एवं उससे अधिक आयु के 1,33,84,332 पुरुषों एवं महिलाओं में सामान्य या आम गैर संचारी रोगों (एनसीडी) की जांच की गई है। एनसीडी की सार्वभौमिक जांच, हाल ही में इस दृष्टि से सामने आए पॉजिटिव मामलों और एचडब्ल्यूसी में इलाज करा रहे लोगों की ताजा स्थिति कुछ इस प्रकार से हैः

राष्ट्रीय स्वास्थ्य नीति, 2017 में व्यापक प्राथमिक स्वास्थ्य सेवा (सीपीएचसी) मुहैया कराने वाले प्लेटफॉर्म के रूप में ‘हेल्थ एंड वेलनेस सेंटरों’ की स्थापना के जरिए प्राथमिक स्वास्थ्य सेवा उपलब्ध कराने की व्यवस्था को सुदृढ़ बनाने पर विशेष जोर दिया गया।
आयुष्मान भारत में दो घटक हैं, जो एक-दूसरे के पूरक हैं। इसके प्रथम घटक के तहत मौजूदा 1,50,000 उप-स्वास्थ्य केन्द्रों (एसएचसी) और प्राथमिक स्वास्थ्य केन्द्रों (पीएचसी) को हेल्थ एंड वेलनेस सेंटर के रूप में तब्दील किया जाएगा, ताकि उपयोगकर्ताओं को व्यापक प्राथमिक स्वास्थ्य सेवा मुहैया कराई जा सके जो सार्वभौमिक एवं निःशुल्क है। इसके तहत आरोग्य (वेलनेस) के साथ-साथ विभिन्न समुदायों को कई सेवाएं उपलब्ध कराने पर भी फोकस किया जा रहा है। इन हेल्थ एंड वेलनेस सेंटरों पर मुहैया कराई जाने वाली विभिन्न सेवाओं में मातृ एवं शिशु संबंधी स्वास्थ्य सेवाएं, संचारी एवं गैर-संचारी बीमारियों का इलाज करने वाली सेवाएं, बुजुर्गों को दी जाने वाली सेवाएं, निःशुल्क आवश्यक दवाओं एवं नैदानिक सेवाओं सहित दर्द से राहत देने वाली सेवाएं शामिल हैं।
